A Maine advance directive is a form instructing medical providers of an individual’s (the principal) treatment wishes. It may also allow an agent, called the principal’s medical power of attorney, to direct health care choices in the interest of the principal. The agent may only act if the principal is unable to make their own decisions.

Signing Requirements (§5-803.2) – Must be signed by the principal and two (2) witnesses.
State Definition (§5-802.1) – “Advance health care directive” means an individual instruction from, or a power of attorney for health care by, an individual with capacity.
